I consent to the retention by CUH&GS of my name, e-mail address, and any other personal information which I may provide upon registration (which information will not be used by CUH&GS for any purpose unconnected with the maintenance of the CUH&GS database, and will not be shared with anyone other than Administrators of the CUH&GS database, the System Administrators of the host-server, and the developer of the TNG software in the event of technical difficulties with the TNG software), and I confirm that all information submitted in this form is true to my best knowledge.
